Nose Landing Gear Actuator Rod End Cracking: How a Known, Supposedly-Fixed Failure Grounded the DA 42 Fleet From Touch-and-Goes
A fractured $211 part. A jammed rudder. A tragic training-flight crash still under investigation. And the unsettling detail: this exact failure, and the exact way crews misread it, was documented thirteen years ago.
Field Notes: On April 29, 2026, a Diamond DA42 (VH-YQP, operated by Flight Training Adelaide) crashed into a hangar at Parafield Airport, Australia, during a simulated one-engine-inoperative training flight, killing both pilots and injuring nine people on the ground. Wreckage examination revealed the nose landing gear (NLG) actuator rod end had fractured from fatigue, separating the actuator from the gear leg.
According to the ATSB's June 23 preliminary report, the NLG remained extended after the main gear retracted, jamming the rudder and causing the aircraft to veer left of runway heading. While the instructor called 'engine failure', the aircraft never climbed past 161 ft AGL, well short of the 400 ft required by company SOPs before simulating an engine shutdown. A 2013 EASA AD documented this exact failure signature, uncommanded yaw and restricted rudder travel after gear retraction, being mistaken for engine power loss in another DA 42. Investigators are still determining whether this crew experienced the same misdiagnosis, or a genuine engine failure called before reaching the SOP altitude.
This isn't the type's first brush with this failure mode. It may be a recurrence of one regulators believed they'd already fixed:
2013 (EASA AD 2013-0121): Traced to insufficient clearance between the rod end safety washer and the NLG attachment lever, causing the rod end to bend at each gear retraction cycle. In the reported incident, that bending was enough to restrict rudder travel and cause an uncommanded yaw which the pilot mistook for an engine failure, but it cleared when the crew re-extended the gear, and the flight landed uneventfully. VH-YQP had this AD's fix installed. In the 2026 crash, the rod end didn't just bend, it fractured completely, permanently separating the actuator from the gear leg. There was no re-extending the gear to clear this one.
2019 (EASA AD 2019-0066): Addressed an in-flight NLG lever fracture that jammed the rudder left until the pilot re-extended the gear, a second, related instance of gear failure disabling rudder control
The accident aircraft had passed its most recent NLG inspection just six weeks prior with zero defects noted.
Regulatory Action: EASA issued Emergency AD 2026-0125-E just six days after the preliminary report, alongside a dedicated Safety Advisory Notice. The FAA adopted it as final rule AD 2026-14-08, effective July 20, 2026.
Until initial inspections are complete, affected DA 42s with over 1,000 hours TIS are prohibited from touch-and-goes and practice go-arounds involving gear cycling. While the accident flight wasn't a touch-and-go, the restriction directly targets the high-cycle fatigue risk driving the fracture.
Why It Matters: The regulatory pace is fast, preliminary report to enforceable U.S. regulation in under four weeks, but the more troubling question is why a fix mandated in 2013, specifically to stop this failure mode, didn't stop it here. The 2013 fix was built around a recoverable failure: rod end bends, rudder jams, crew re-extends the gear, and lands safely. The 2026 crash shows the same root defect can progress further, the rod end fracturing completely, with no re-extension able to clear it. Until root-cause analysis explains that escalation, "already fixed" can't be assumed to mean "can't get worse." For now, 267 U.S.-registered aircraft are affected, some with inspection windows as tight as 5 flight cycles. A clean logbook entry from six weeks ago wasn't enough to catch this one.
