A rapid nose-down pitch change. A replacement part that turned out to be just as dangerous as the original. And an emergency grounding that gave operators a strict 3-day window just to fly to a repair station.
Field Notes: In April 2003, the FAA issued emergency AD 2003-06-51 after a Learjet 45 experienced severe vibration and an uncommanded pitch change in flight. The cause was traced to a brittle fracture in the horizontal stabilizer actuator assembly (HSAA), the part driving pitch trim. Operators were required to swap the suspect assembly for an approved replacement (P/N 6627401000-005).
Four months later, the twist came: the FAA discovered that the replacement -005 part wasn't safe either. Made from the same material as the failed component and plagued by manufacturing quality deficiencies, the FAA lost confidence that the airplane could fly safely for any period of time. The resulting AD (2003-16-19) grounded every Learjet 45 with a -005 actuator installed, effective immediately on August 13, 2003, permitting only single positioning flights within a 3-day grace period until an approved new HSAA part was installed.
Why This Still Matters Today: This isn't just a story about a 20-year-old part failure, it's a great example of why component-level logbook tracking matters. When an AD requires swapping a specific part for a modified variant, "AD 2003-16-19 complied with" on its own doesn't cut it decades later. An auditor or buyer in a pre-buy needs the exact part and serial numbers on file to confirm the aircraft isn't still flying on a recalled -005 actuator, because on paper, full compliance and a mere sign-off look identical until you trace the numbers through the historical record. AD compliance is only as good as the paper trail behind it.
